Personal Details

You'll need to provide your full name (including first name, surname, usual name, any given names, birth surname if married or since changed), other names or aliases, home address (or city and country of residence if no address is available), contact phone number(s), valid email address, date, place, and country of birth, current nationality, other nationalities (if applicable), first names of your parents, education history (from primary school to higher level or none), gender, and occupation. Additional information may be needed about your job title, job description, and employer details. Students are required to provide details of their school or college​​.

Documentation and Travel Plans

The form requires details of your current passport or travel document, including the document number, type, country of issue, issue and expiry date. You must also provide the name of the first member country you plan to visit and possibly the address of the hotel or accommodation where you will spend the first night(s). For those applying for the first time for a specific holiday or business trip, the intended destination country and accommodation address are required, though this is not necessary for subsequent trips​​.

Security Checks

A section on the form is dedicated to your criminal record and potential security risks. You must disclose any criminal convictions in the past ten years, including for sexual crimes involving children, human trafficking, drug trafficking, rape, murder, money laundering, criminal damage to persons or property, and terrorism (in the past twenty years). You will also need to provide details of any deportation order from any European member country or a “third country” that requires a visa to enter the European zone, as well as any stays in or visits to specified war zones or areas of conflict in the last ten years​​.

Health Checks

You'll be required to provide information about any existing medical conditions, including infectious diseases, contagious parasitic diseases, and serious illnesses​​.

Minors

For minors seeking ETIAS approval, the form is similar to that for adults, but it must be filled out by the parent or guardian responsible. It requires additional information, including the name, address, email, and phone number(s) of the parent or legal guardian, and the relationship of the applicant (son, daughter, etc.)
These requirements reflect the ETIAS's goal of increasing security throughout the European Union by thoroughly vetting travelers before they arrive.

How to pay ETIAS online application form?

To apply for ETIAS, there is a fee of 7 euros. This fee must be paid electronically using a debit or credit card to finalize the application. After the payment is processed, most applicants will quickly receive approval, typically within minutes. Once approved, the ETIAS visa waiver remains valid for a period of three years from the date it is issued.
